A 21-year-old male had an accidental fall from height while working near Azam Shah Chowk, Central Avenue at around 1:35 pm on 19 Aug 2023 followed by penetration of two 6 feet iron rods of 16 mm diameter in chest and abdomen.
He presented at SevenStar Hospital emergency department with 2 penetrating Rod Rods, one penetrating from left side of chest coming outside from just near to heart and one rod penetrating from left side of abdomen coming out from right side of pelvic region with profuse bleed.
The injury looked “unsurvivable”, patient was seen by doctors in emergency department, the trauma mechanism and injury kinetics were analyzed, basic resuscitation was done, blood was arranged, without any delay, decision to shift patient in OT was taken, the size of rod was too big, it was difficult to move patient, patient cannot be shifted in lift, machine was called and rods were cut immediately so that patient can be moved to the OT at the earliest.
Surgeon’s along with OT team swung into the OT, the rod was still big and had to cut again in OT covering the patient properly as it was difficult for surgeons to stand for surgery.
Abdomen was opened by surgeon, chest rod entered from thorax pierced diaphragm, just touching the liver but didn’t pierced the liver, the liver was not shattered and the team was able to save the liver. The chest rod pierced lung, diagphragm, went to rib cage, then came out from other side narrowly missing liver, bowel and came out very close to heart.
The other rod pierced from left side of lower abdomen, it went from back side missing the bowel, injuring the hip bone. It was very difficult to remove the lower rod as it was fixed tightly and patient’s cloth was fixed inside the bone along with the rod. It has to be hammered from the opposite side by orthopaedic surgeon, with great difficulty rod was finally removed.
Hip bone was shattered into pieces and it was bleeding heavily, which was taken in control by surgeonsand the surgery was finally successful saving all vital organs of patient.
CT Abdominal Angiography and 3D CT bony pelvis post operatively confirmed successful surgical intervention.
The patient was on ventilator in ICU for 2 days and weaned off ventilator as he was doing very good under the care of ICU team.
The patient had an uneventful further recovery in ward. All surgical wounds healed well and there were no post operative complications.
The patient was successfully discharged from hospital on 1 September 2023.
